Live Well, Be Well Home

LifestyleThe Live Well, Be Well Program was a community-based lifestyle diabetes risk reduction program designed for lower socioeconomic status, minority, and Spanish-speaking adults at risk of developing diabetes.  Primarily telephone-based, participants received education and skills training to improve diet and increase physical activity.  We provide a program manual and participant program binder.  Program materials are available in English and Spanish.

 
Outreach

 

Community-based outreach and diabetes screening in lower-income communities provided health education about diabetes risk factors (using the Diabetes Risk Appraisal (DRA)) and personal risk.  Outreach and diabetes screening is an optional component that can be done with or without the Live Well, Be Well program.

 

 

The Live Well, Be Well program was evaluated through a 12-month randomized controlled trial which compared the lifestyle program group to a wait-list control group.

 

The project was a partnership between the University of California San Francisco (UCSF) and the City of Berkeley Division of Public Health.  It was funded by a translational research grant (R18 DK067896) from the National Institute of Diabetes and Digestive and Kidney Diseases.
